I think it is a tactical decision. If your life is at risk Right Now (i.e. a gun in your face), you're going to draw and fire. Short of shooting the wrong person, you probably aren't going to make things much worse.
But if you're in a situation where someone else is threatened, you may have the luxury of time to evaluate the situation - how many shooters, are they good guys or bad guys, is drawing attention to yourself (just by drawing) going to change the balance/timing of the situation, will doing this maybe make me look like a bad guy to other good guys, etc.
i.e. you're effectively in condition orange.
And you should ALWAYS assume there is another shooter and keep scanning (with your eyes).
Worth reading is the story of the CWP holders in the Giffords shooting and the guy who had taken the gun off the shooter. Really worth thinking about.
